 This fillable .pdf form includes necessary school, coach and team member information - e-mail this to the BCB Coordinator!  The Coach signs an agreement regarding guidelines for participation - mail this to the BCB Coordinator.



 

2010 Reg Packet Cover Letter & Instructions

Coach of Record completes and signs and submits this with the Team Reg Form by Jan. 22, 2010
Instructions for compiling the Registration Packet, BCB Coaching & Team Participation Agreement to be signed. Mail this to the BCB Coordinator, along with other Registration Forms.




 

2010 Student Participation Forms

Student guardian completes & signs three forms,

Coach of Record submits these by Jan. 22, 2010

(1) Consent for student to participate in NOSB competitions, regional and/or national; (2) Student medical/emergency forms for BCB and NOSB Coordinator use only; (3) Consent for student to be interviewed and photographed, acknowledge NOSB right to publish.
